Healthcare receives 80% of the health dollar and effects 20% of health outcomes. This session will describe two efforts to increase healthcare's impact by implementing social needs screening, community navigation and aligning the community around gaps in social needs. Implementation learnings, preliminary outcomes and a vision for the future will be discussed.
