The Regional Health Connector (RHC) Program in Colorado strives to created a more connected health system in order to address community health needs. Through the EvidenceNOW Southwest project, we had the ability to evaluate the RHC Program from the perspective of the primary care practices. We found that a closer relationship with an RHC was associated with a higher ability for practices to link patients to community resources and to address their social needs.
