Patients experiencing homelessness represent a vulnerable population affected by substantial health inequities. We established a Community Advisory Panel comprising individuals with lived experience of homelessness, and key community stakeholders in order to better understand their perceptions regarding hospitalization and care transitions, and to develop a shared vision for improvement. Salient themes and implications for healthcare providers, organizations and policy stakeholders are discussed.
