Our presentation will provide an overview of lessons-learned from an iterative process of engaging community members, academicians, researchers, and patients in a four-phase project aimed at identifying the facilitators and barriers to sustaining patient-centered outcomes research partnerships. These identified facilitators and barriers to partnership sustainability have been used to collaboratively build a partnership sustainability toolkit framework, develop manuscripts, and create an infographic, all of which are designed to provide best-practices around sustaining community-academic partnerships.
