Early Perspectives on SDoH Screening and Referral Implementation in Community-Clinical Linkage Programs

The Partners in Evaluation and Research Center at Kaiser Permanente Colorado evaluated the planning and implementation process for social determinants of health screening and referral within clinical programs. 7 clinical programs successfully implemented screening and referral and provided feedback on their facilitators and barriers. Clinical programs incorporating screening and referral for social needs should consider resource availability and the opportunity cost of the process, and prioritize building and maintaining collaborative partnerships with referral organizations.
