From 2016-18, CDPHE’s Cancer, Cardiovascular, and Pulmonary Disease (CCPD) Grant Program funded 47 grantees to policy and environmental changes, community clinical-linkage programs, and health systems transformations to improve health behaviors and health outcomes. This presentation will review the purpose of CCPD, the evidence-based strategies that were funded, and the findings from the cross-site evaluation. The presentation will include a discussion of how evidence-based strategies can be strengthened using an equity lens to improve population health.
