The Colorado Public Health Association and the Colorado Department of Public Health and Environment’s (CDPHE) Prevention Services Division, is offering a free pre-conference workshop for Colorado’s public health professionals on Tuesday, August 27th at the Keystone Lodge & Conference Center. Welcome and breakfast will be from 8:15am-9:00am. The pre-conference workshop will go from 9:00am-4:00pm.
This day-long interactive workshop will include:
- Opening Presentation and Dialogue. Socially Determined: Moving to Public Health 3.0: Outline of an Implementation Roadmap for Chronic Disease Programs
- Panel Discussion. Current Evidence-based Practices and Innovations in Chronic Disease: Disruptions that Make an Impact (LPHA, State and CBO partners discussing the linkages they make in their work between chronic disease and determinants of health)
- Presentation and Dialogue. The Landscape of Plans and Priorities and Integrating Chronic Disease
- Listening and Working Session. Developing Plans and Priorities That Make an Impact: Linkages Between Social Determinants and Chronic Disease Prevention and Management
